The patient is relieved, you're focused \u2014 and the chart awaits. But will you have time to document everything before the next patient is in the chair?<\/em><\/p>\r\n\r\n\r\n<hr class=\"wp-block-separator has-alpha-channel-opacity\" \/>\r\n\r\n\r\n<p>Record keeping isn't the first thing most dentists think about when describing their workday. Yet, there are few places in the healthcare system where documentation has such direct consequences as in the dentist's chair. Inadequate records can mean that the Health Supervisory Authority cannot assess whether the treatment was appropriate. It can mean that a compensation case tips in the patient's favor. In the worst case, it can cost you your license.<\/p>\r\n\r\n\r\n\r\n<p>At the same time, everyone working clinically knows that the time between patients is scarce. The medical record competes with X-rays, material selection, treatment plans, and the next patient who is already waiting. The question is not whether documentation is important \u2013 it\u2019s how you can manage to do it well enough.<\/p>\r\n\r\n\r\n\r\n<h2 class=\"wp-block-heading\">The Duty to Document: Stricter Than Many Realize<\/h2>\r\n\r\n\r\n\r\n<p>All dentists are familiar with Section 39 of the Health Personnel Act, which states that anyone providing healthcare must keep a journal. However, it is Section 40 and the regulation on patient records that set the specific requirements \u2013 and they are more extensive than many are aware of.<\/p>\r\n\r\n\r\n\r\n<p>The journal shall contain relevant and necessary information about the patient and the healthcare provided. In practice, this means: the purpose of the examination, objective findings and subjective symptoms, which examinations and X-rays were taken, diagnosis and treatment provided, materials and equipment used, and any complications. In addition, it shall be documented what information the patient has received, and any referral to another healthcare provider.<\/p>\r\n\r\n\r\n\r\n<p>Statens helsepersonellnemnd has clarified that brief, summary-style journal entries are against the law. The medical record should provide a clear and comprehensive overview \u2013 not just for yourself, but for any other healthcare professional who might take over the patient's care.<\/p>\r\n\r\n\r\n\r\n<h2 class=\"wp-block-heading\">When the journal is not up to par<\/h2>\r\n\r\n\r\n\r\n<p>The Norwegian Board of Health treated in 2025 the entire <strong>390 new supervision cases<\/strong> for healthcare professionals and businesses, and gave <strong>220 reactions<\/strong> \u2014 down from the 131st year before. Among them: two dentists who had their authorization revoked. Insufficient record-keeping is a recurring issue in supervisory cases against dentists. In a well-known case, a dentist received a warning after prolonged failure in record-keeping, combined with incorrect reimbursement claims to the state. The records lacked descriptions of professional assessments, X-ray findings were not documented, and there were no contemporaneous notes about treatment choices.<\/p>\r\n\r\n\r\n\r\n<p>The consequences of poor record-keeping are not theoretical. The Supreme Court ruled as early as 1989 that doubt in evidence resulting from inadequate records shall be weighed against the practitioner. If you cannot document that the treatment was appropriate, you bear the burden of proof. For dentists, who often perform irreversible procedures, this is a particularly important point.<\/p>\r\n\r\n\r\n\r\n<p>It's not just about supervision and law. A good chart is also your most important tool for ensuring continuity of treatment \u2014 especially in long-term cases like orthodontics, implants, or periodontal follow-up.<\/p>\r\n\r\n\r\n\r\n<h2 class=\"wp-block-heading\">Dental health in digital transformation<\/h2>\r\n\r\n\r\n\r\n<p>The dental health service is undergoing a digital transformation. From January 1, 2021, all practicing dentists must have electronic patient records (EPR) \u2013 partly because it is impossible to send Helfo reimbursement claims without electronic settlement. The era of paper records is definitely over.<\/p>\r\n\r\n\r\n\r\n<p>Now the transition is taking a new step. Through the DigiDOT project, all county municipalities are collaborating on a common, modern patient record system for the public dental health service. The procurement was announced in March 2026, and the goal is clear: safer patient treatment, better collaboration with the rest of the healthcare service, and easier access to information for both patients and practitioners.<\/p>\r\n\r\n\r\n\r\n<p>But even the best charting system doesn't solve the fundamental problem: someone has to write the notes. And that's where daily life pressures.<\/p>\r\n\r\n\r\n\r\n<h2 class=\"wp-block-heading\">From the treatment chair to the patient record \u2014 without losing flow<\/h2>\r\n\r\n\r\n\r\n<p>The challenge for dentists differs from many other healthcare professionals. You work with your hands, often in situations where you need full concentration on the patient. Stopping mid-procedure to take notes is rarely realistic. The result is that the patient record is written afterward \u2014 often in a compressed form, between patients, or in the worst-case scenario at the end of the day when the details have already begun to fade.<\/p>\r\n\r\n\r\n\r\n<p>Medivox is developed to solve precisely this. At the same time, the Norwegian Directorate of Health's 2025 healthcare personnel survey shows that speech-to-text tools are rapidly growing among healthcare professionals \u2014 with general practitioners leading the way, where <strong>61 %<\/strong> already have access to such tools.<\/p>\r\n\r\n\r\n\r\n<p>Dentists haven't been among the first to adopt this technology.
