It's the one that answers the question: \u00abWhat happened with the patient today, and what will happen next?\u00bb<\/em><\/p>\n<hr \/>\n<p>You've just finished a consultation. The patient left a minute ago, you have four patients left before lunch, and now you're sitting in front of a blank notepad. What exactly is it that <em>may<\/em> stand there \u2013 and what is it that just <em>habit<\/em> to stand there because it always has?<\/p>\n<p>For many general practitioners, the medical record note is one of the more time-consuming tasks than it needs to be, but also one of the less enjoyable tasks to discuss at a professional seminar. It's rule-governed, it's routine, and it's often poorly documented how other doctors actually handle it. Nevertheless, a good note protects both the patient and you, and it shortens the next consultation. A bad note does neither.<\/p>\n<h2>What the law requires \u2013 and what it does not require<\/h2>\n<p><a href=\"https:\/\/www.helsedirektoratet.no\/rundskriv\/helsepersonelloven-med-kommentarer\/dokumentasjonsplikt\/-40.krav-til-journalens-innhold-m.m\" target=\"_blank\" rel=\"noopener\">Healthcare Personnel Act \u00a7 40<\/a> states that the journal shall be kept in accordance with good professional practice and contain <strong>relevant and necessary information<\/strong> of the patient and healthcare. It must also be easy to understand for other qualified healthcare personnel. The law does not require a specific heading, a specific order, or a specific number of words. It requires that the content is there.<\/p>\n<p><a href=\"https:\/\/lovdata.no\/dokument\/SF\/forskrift\/2019-03-01-168\" target=\"_blank\" rel=\"noopener\">The Patient Journal Regulations<\/a> specifies what is often included: the patient's condition and reason for contact, examinations performed, current diagnosis, treatment and care, medications, referrals and evaluation of referrals, advice and information provided, as well as information about communication, reservations, and decision-making capacity where applicable. The list is not exhaustive \u2013 it is a checklist of what <em>can<\/em> be relevant.<\/p>\n<p>It's not about writing as much as possible. It's about writing what is necessary for a colleague to be able to take over tomorrow and understand what's happening, and for you yourself to be able to document for the patient, a relative, or a potential review case what was considered and why.<\/p>\n<h2>A structure that holds regardless of consultation<\/h2>\n<p>Most Norwegian general practitioners use a variant of <strong>SOAP<\/strong> \u2013 or the Norwegian translation <strong>AUVT<\/strong>:<\/p>\n<ul>\n<li><strong>A<\/strong>Subjective - What the patient says, reason for contact, symptoms, and duration<\/li>\n<li><strong>U<\/strong>Examination \/ Objective - what did you find; measurements, clinical findings, any test results<\/li>\n<li><strong>V<\/strong>Ordering \/ Assessment \u2013 what do you think clinically; differential diagnoses, severity<\/li>\n<li><strong>T<\/strong>iltak \/ Plan \u2013 what was done or ordered; follow-up, sick leave, referral, next contact<\/li>\n<\/ul>\n<p>The structure is not a legal requirement, but it precisely solves what the law requires: a colleague should be able to read the note and understand the context. It also helps you yourself not to \u00abforget\u00bb anything under stress \u2013 for example, to write down what was <em>considered and rejected<\/em>, not just what was chosen. A differential diagnosis you considered and ruled out is often just as important as the one you chose.<\/p>\n<p>A brief consultation for a patient with known asthma can be summarized in four short lines following this structure. A complex assessment of a patient with unclear chest pain may well take twenty. The length of the note should follow the complexity of the case, not a standard length.<\/p>\n<h2>The little details that often slip away<\/h2>\n<p>Even a well-structured memo has some classic pitfalls. <a href=\"https:\/\/www.helsetilsynet.no\/globalassets\/opplastinger\/publikasjoner\/tilsynsmelding\/2005\/mangelfull_dokumentasjon_pasientopplysningar.pdf\" target=\"_blank\" rel=\"noopener\">The Norwegian Board of Health Supervision's inspection report on deficient documentation<\/a> And a long series of specific supervision cases over the years point to the same recurring weaknesses: test results that have been received but not acknowledged, follow-ups that have been planned but not logged, phone calls that never made it into the medical record, and medications prescribed without justification.<\/p>\n<p>Some of the details that are easy to forget, but that supervision and patient complaints often deal with:<\/p>\n<ul>\n<li><strong>Reason for contact in the patient's own words.<\/strong> \u00abChest pain\u00bb and \u00abthe patient says it stabs in his chest when he breathes deeply\u00bb are not the same.<\/li>\n<li><strong>Time spent and timing.<\/strong> How long have the symptoms lasted, and what has happened since last time?<\/li>\n<li><strong>Patient Information and Consent.<\/strong> What was the patient told? What did the patient consent to - and what did she reserve her opposition to?<\/li>\n<li><strong>Justification for choice.<\/strong> Why sick leave now and not next week? Why a referral now and not a wait-and-see approach?<\/li>\n<li><strong>Who has done what.<\/strong> And <a href=\"https:\/\/lovdata.no\/dokument\/HRSIV\/avgjorelse\/hr-2026-372-a\" target=\"_blank\" rel=\"noopener\">Supreme Court judgment from 2026 (HR-2026-372-A)<\/a> emphasizes that the medical record must make it possible to identify who has made each entry \u2013 full name is the general rule, but initials can be used as long as the patient is informed of who made the entry upon inspection.<\/li>\n<\/ul>\n<p>A note covering these points is rarely long, but it is solid. It's the kind of note you're glad to have written if the patient calls a colleague three days later - or if Helfo, the Norwegian System of Patient Injuries, or the patient themselves requests access.<\/p>\n<h2>How much time do you actually spend on the journal?<\/h2>\n<p>Norwegian general practitioners <a href=\"https:\/\/www.fhi.no\/publ\/2023\/arbeidsmengde-konsultasjonstid-og-utilsiktede-effekter-av-takstsystemet\/\" target=\"_blank\" rel=\"noopener\">The weekly working hours have increased from 36 hours in 2008 to 41 hours in 2021.<\/a> \u2013 while the number of consultations per inhabitant has remained stable at <a href=\"https:\/\/www.helsedirektoratet.no\/rapporter\/arbeidet-med-allmennlegetjenesten\/befolkningens-bruk-av-fastlege-og-legevak\/konsultasjoner-med-fastlege\" target=\"_blank\" rel=\"noopener\">3.6 times per year in both 2023 and 2024<\/a>. A simple check may suffice with a few lines; a complex assessment needs more.
