Four professional groups work with muscles and skeleton, have hands as their most important tool \u2013 and encounter the same wall when a consultation needs to become a journal entry. The rules surrounding them are different. The challenge is the same.<\/em><\/p>\n

The four professions each have their own history within the Norwegian healthcare system, and they have different regulatory statuses today. But place them side by side in a busy clinic day, and the similarities become clearer than the differences: They treat with their hands, they work to tight schedules, and they have a documentation obligation that cannot always be reconciled with a keyboard.<\/p>\n

It's worth looking at what separates them\u2014and what binds them together.<\/p>\n

Different path in, different role<\/h2>\n

The chiropractor<\/strong> has the longest tenure as authorized healthcare personnel and a broad role. The chiropractor is the primary contact for musculoskeletal complaints, can refer to medical specialists, order imaging diagnostics such as X-rays, CT, and MRI, and provide sick leave certificates for up to twelve weeks. These rights are anchored in Regulation on subsidies for expenses for chiropractic treatment<\/a> and among other things assumes a social security course.<\/p>\n

Manual therapist<\/strong> \u2013 a physiotherapist with further education in manual therapy \u2013 has a corresponding set of expanded rights: referral, requisition, and sick leave within the musculoskeletal field. Osteopath<\/strong> and chiropractor<\/strong> are the newest in the authorization scheme. With the amendment that granted naprapaths and osteopaths authorization as healthcare professionals<\/a>, they went from being regulated by the alternative treatment act to being subject to the Health Personnel Act. From May 2023, the titles are protected \u2013 if you use them, you are authorized health personnel with all the duties that entails. They do not have the same set of referral and sick leave rights as chiropractors and manual therapists, but they have a full duty to keep records.<\/p>\n

Five professions, then, on a scale from broad gatekeeper role to recently licensed practitioner. Yet they end up with the same basic documentation requirements.<\/p>\n

The common duty: the journal must stand firm<\/h2>\n

No matter where you are on the scale, the same core applies: The Directorate of Health is clear that the journal must contain the necessary and relevant information.<\/a>, and that the therapist is responsible for the content.<\/p>\n

For the newly authorized osteopath or naprapath, this in itself is an adjustment. The duty to keep records according to the Health Personnel Act is more formal and more extensive than what many were used to under the old regulations. History, findings, assessment, and treatment must be documented so that another practitioner \u2013 or a supervisory authority \u2013 can understand what was done and why.<\/p>\n

For the chiropractor and manual therapist, there is an extra layer on top: Every referral, every prescription, and every sick note must be justifiable based on the patient's record. When you make a decision that has traditionally been the GP's responsibility, the note must show that the decision was professionally well-founded.<\/p>\n

The result is the same for all four: the journal is not a formality you fill out afterward. It is part of professional responsibility \u2013 and for the professions that refer and issue sick notes, part of the very foundation for the system to continue.<\/p>\n

My hands are full \u2013 and that's where the problem lies<\/h2>\n

This is where the four of them meet in earnest. A manual therapist cannot write while they are working. Their hands are on the patient \u2013 that is the whole point of the profession. The history is taken while you palpate, the findings are made while you move the joint, the assessment is formed while you treat. The actual work and the documentation of the work compete for the same two hands.<\/p>\n

The traditional approach is to write the notes afterward\u2014between patients or at the end of the day. And that is precisely where the documentation becomes less thorough than it should be. Details get left out. The red flag you thought of but didn\u2019t have time to write down. The functional assessment behind the sick leave, which becomes \u00absick leave 100 % for 14 days\u00bb without the rationale. When the conversation can be captured without writing<\/h2>\n

A patient history is primarily a conversation. The patient describes the pain, how it started, what makes it worse, and what makes it better. The examination is commented on out loud \u2013 \u00abthere is reduced mobility here,\u00bb \u00abtenderness on palpation here,\u00bb \u00abnegative test there.\u00bb Much of what the record needs is actually said aloud in the room. This means it can also be captured without you having to let go of the patient to write.<\/p>\n
